Volunteer Safety Guidelines Guidelines for holding a safe and successful adoption-related cleanup. Volunteer Waiver/Parental Consent Form A volunteer waiver is required from each volunteer prior to participating in a PA CleanWays adoption-related event. The parental consent portion of the form is required for each youth younger than 18 desiring to volunteer in an adoption cleanup. Adoption Cleanup Reporting Form Form for reporting results of adoption cleanup - to be sent within 2 weeks of event.
